When you book an appointment or become a patient, we collect information such as your name, contact details, date of birth, and clinical records (assessments, treatment plans, and progress notes) needed to provide your care.
Your information is used to schedule and confirm appointments, maintain your clinical record, communicate with you about your care, and improve our services. We do not sell your personal information.
Only authorized clinic personnel involved in your care — your assigned therapist, head personnel, and system administrators — can access your clinical record.
